Your primary care physician’s name, address, and phone number.
Step 2:
Speak with SelectHealth to confirm your eligibility Monday — Friday, 8 am — 6 pm.
Step 3:
Once your eligibility is verified, you will be connected to the New York State Marketplace or the New York Medicaid Choice Hotline.
If you do not have your Medicaid card, you will be asked to provide the following: your first name, last name, date of birth, and Social Security number.
To keep your Medicaid active, you must complete the re-certification form that you received in the mail. To re-certify your Medicaid coverage, you can contact the NY Medicaid Choice hotline at 1-800-505-5678 or locate your closest Medicaid office.
If you enrolled in Medicaid through the Department of Social Services, call NY Medicaid Choice at 1-800-505-5678. If you enrolled in Medicaid through NY State of Health, please call 1-855-355-5777.
Yes. NY Medicaid Choice can also route calls to the Marketplace if needed. NY Medicaid Choice is the best first step for all new potential enrollees.
