What if my address or telephone number changes?

If your home address or other contact information changes, please call us at 1-866-469-7774 (TTY: 711) to update your contact information. If your new address is within our service area (Brooklyn, the Bronx, Manhattan, Queens, Nassau County, or Westchester County), your coverage will not be affected. You will also need to contact your local department of social services so they can update your information:
