Contact the SelectHealth Care Team at 1-866-469-7774 (TTY: 711). If you have already chosen a new doctor, give us the doctor’s name. Once we confirm that the doctor participates with SelectHealth, we’ll send you a new SelectHealth Member ID Card.
If you have not chosen a new doctor, you can find one by using our Provider Search tool or asking a SelectHealth Care Team representative to help you find an in-network Primary Care Physician.
